Born in Cleveland, Sam earned her PharmD from Ohio Northern University and completed her residency at the Huntington VA Medical Center. For the past decade, she served family medicine patients across West Virginia Appalachia, specializing in transitions of care, chronic disease management, medication therapy management (MTM), and helping patients access affordable medications through financial assistance programs.

In 2025, Sam began her DPC-focused journey with a clear mission: to give Direct Primary Care providers embedded clinical pharmacy service lines that mirror the support found in traditional family practice—without adding workload to the team. Her services are delivered 100% virtually and are not limited by state borders, helping DPC providers improve outcomes while protecting their time.

Sam lives in West Virginia with her husband and two kids and enjoys white water rafting the New & Gauley river, hiking trails that include any size waterfall, traveling to beaches that have a clear view of marine life, and any time spent with her enormous family.
